Walton consent form

Signed before a minor is examined or treated. One page, and it saves time at the desk.

Georgia requires a parent or legal guardian to consent before a minor receives care. For team and school programs we ask for it in writing in advance, so that a player who turns up injured on a Friday is not waiting on a phone call.

Who needs to sign

A parent or legal guardian of any patient under eighteen. One form covers the season rather than each visit.

What it covers

Consent for examination and chiropractic care, your contact details, and anything in the child's history we need to know before we touch them — previous injuries, current medication, recent concussion.

If a child arrives without it, we will still see them in an urgent situation and take verbal consent by phone. The form simply removes that step.
